چکیده

Hypothermia is a condition wherein the body temperature drops below 35°C when an individual is unconscious. In prolonged moderate hypothermia (28–32°C), progressive drops in the pulse rate and increases in atrial and ventricular arrhythmia and Osborn waves are observed in electrocardiography (ECG). Unconsciously, staying outside for an extended time in extreme cold is the most important cause of hypothermia. In recent years, a significant social problem is the increase in the number of people using synthetic cannabinoids (SC) in public places. A frequent result of SC use is severe deterioration in consciousness and perception. A 22-year-old male patient, who was found unconscious in a park and was brought to the emergency department by 112 emergency services, was reported to have used SC. During vital sign examination, body temperature was too low to be measured, SpO2 level was 93%, and the other parameters were normal. There was no obvious sign of injury on his body. The initial assessment led us to believe that the patient was in a hypothermic condition because of unconsciousness after drug use and exposure to cold. Atrial fibrillation and giant Osborn waves were observed in his ECG (Fig.1, 2). Four hours later, the body temperature increased to 35.7°C. ECG performed for control revealed that Osborn waves disappeared, and the rhythm returned to a normal sinus rhythm (Fig. 3). Although there was no thermometer that measures body temperature below 35°C in our department, typical Osborn waves observed in ECG led us to consider that the body temperature was between 28 and 32°C and to intervene in the patient early. Osborn waves of hypothermia.

A12-lead ECG was obtained from a 72-year-old man with a core body temperature of 85°F (Figure 1). The ECG shows severe sinus bradycardia with prolonged PR, prolonged QRS complex, prolonged QT interval, and an extra deflection at the end of the QRS complex (Osborn waves) (arrows). A second 12-lead ECG obtained 24 hours later, after the patient had been rewarmed to 98°F, was normal (Figure 2). Hy...

Objective: Patients with hypothermia are frequently admitted to the intensive care unit. Hypothermia may be associated with typical ECG changes (Osborn waves). Here, we present a typical case report with sequential ECG examinations from hypothermia to normothermia. Design: A case report. Patient: A 56-year-old woman was hospitalized with prolonged hypothermia after a suicide attempt with oxazep...

Osborn Waves: History and Significance

The Osborn wave is a deflection with a dome or hump configuration occurring at the RST junction (J point) on the ECG (Fig. 1). In the historical view, different names have been used for this wave in the medical literature, such as “camel-hump sign”, “late delta wave”, “hathook junction”, “hypothermic wave”, “J point wave”, “K wave”, “H wave” and “current of injury”.1 Although there is no defini...

ECG Changes Due to Hypothermia Developed After Drowning: Case Report

Drowning is one of the fatal accidents frequently encountered during the summer and is the most common cause of accidental death in the world. Anoxia, hypothermia, and metabolic acidosis are mainly responsible for morbidty. Cardiovascular effects may occur secondary to hypoxia and hypothermia. Atrial fibrillation, sinus dysrhythmias (rarely requiring treatment), and, in serious cases, ventricul...

Atrial fibrillation following synthetic cannabinoid abuse.

Marijuana and its synthetic forms, called synthetic cannabinoids (SCs), are used as recreational drugs. Bonzai is a kind of SC. Adverse cardiovascular events have been reported with abuse of marijuana and SCs, including arrhythmia, myocardial infarction, and sudden cardiac death. Presently described is a case of a 23-year-old, previously healthy man, who was admitted to the emergency department...
